LIST YOUR CHILD’S NAME AND GRADE AND TEACHER ON THE NOTE, TOO.
The doctor’s note must include the food/foods to be omitted.
It must have also include foods to be substituted – including BRANDS.
(For example-there are many different types of hot dogs. We need to know the brand that is safe for your child. If possible, we will get it for your child. Sometimes items cannot be obtained, have a short shelf life or are too expensive for the program to purchase.)
BE SPECIFIC AS TO WHAT THE CHILD CANNOT HAVE. WE WILL WORK WITH THE NURSE AND YOU TO SEE THAT THE CHILD HAS AN ALTERNATIVE SELECTION, IF POSSIBLE. IF YOU CAN COME UP WITH ALTERNATIVES, PLEASE FEEL FREE TO SUGGEST THEM TO THE STAFF IN WRITING (BE REASONABLE, PLEASE).
PLEASE SEND A COPY OF THE NOTE FROM THE DOCTOR TO THE CAFETERIA. WE NEED THIS FOR OUR RECORDS. PUT YOUR CHILD’S GRADE AND TEACHER ON THE NOTE, TOO.
WE WILL SEE THAT YOUR CHILD CAN CHOOSE EITHER A SOY MILK OR LACTAID MILK.
THE RULES HAVE CHANGED AND WE CAN NOT SUBSTITUTE JUICE FOR MILK. STUDENTS HAVE TO BE OFFERED A CALCIUM RICH FOOD - THEY DO NOT HAVE TO TAKE IT.
IF THEY CAN STILL HAVE A LITTLE DAIRY SUCH AS ICE CREAM OR CHEESE, PLEASE INCLUDE THAT ON THE NOTE SO THE CASHIER WILL KNOW. OUR NEW POINT OF SALE CASH REGISTERS WILL HAVE THE CAPABILITY OF TELLING THE CASHIER ABOUT FOOD ALLERGIES.
